Welcome to the Practice Managers desk online.
This area is designed to make it easier
to answer questions about our facility when the doctor recommends
aquatic or land-based physical therapy to your patients. If you
are comfortable with a facility that has established a history
with your office, we urge you to visit our Co-Care
Alliance webpage where
your concerns will be put to rest.
Patient referrals are important as they can make or break your
credibility with your patients. We vow that you will never be disappointed
with how your patient is received and cared for when in our facility.
The intention is to make it almost effortless to handle the referral
process and we urge you to feel free in connecting them directly
to our front desk to handle the bulk of the process.
As part of our concierge desk you will be hearing from us when
we make follow-up appointments to your office on behalf of your
referred patients. The process is an easy one as we will usually
ask for three choices of time and date while the patient is in
front of us with their calendar.
This service is provided so that we may synchronize the follow-up
visit with our re-evaluation schedule. If we are co-treating with
another facility or hospital, we handle the synchronization process
with them before engaging in a follow-up appointment with you.
We have taken the liberty in posting some useful links that you
can use as a quick reference that may be of interest to you. They
are listed in the Interesting articles and links area of the Doctors
lounge.

Information we need from you to make the process efficient includes:
- A Doctors prescription with DX Code/s, frequency,
duration of therapy and special instructions or we will be
delighted to provide you with our referral pad, which contains
prompts for this information.
- Please note Medicare Patients
require a new prescription every thirty days. You will receive
a re-evaluation no later than 28 days into the cycle.
- Your
office telephone and fax numbers and address
- Please send a
few of the Doctors business cards.
- If your patient asks if we
are experienced with their particular condition, you may check
that against the following conditions most familiar to us.
Being an organization with many capable relationships, if we
cannot assist in our facility we will direct them to specialists
through our referral network. This also works well for "snow
birds" and patients who simply live too far from us.

The main entrance is the door at the parking
lot.
Items needed for first appointment:
- You need a current prescription and/or a referral from a physician,
depending on your insurance carrier and plan.
- Please have two forms of Identification. This is for your protection
Insurance card/s and form of payment
for co-pays or co-insurance.
If you have downloaded and completed our intake
forms, please bring them with
you, if you have chosen not to download the forms, we ask that
you arrive 15
minutes prior to your appointment to complete out intake information.
Bring your calendar or planner if you have
one. So you may set-Up your
schedule.
Wear comfortable clothes and shoes. If coming for a hip, knee
or ankle injury
please brings shorts.
Insurance:
Please note that we are constantly in application to be in-network
with many insurance plans. Sometimes the guide is not printed for
months after we enter contract and websites do not always update
in a timely fashion. We have listed the insurance companies we
have dealt with but will handle the pre-certification process to
assure there are no problems for your patient.

Forms Of Payment Accepted:
For co-pays, Co-insurance and deductible payments at time of service,
we are pleased to accept: Cash, Personal Checks, MasterCard, Visa
or Discover.
Cancellation Policy:
We require 24 hours notice for intentional cancellations. If an
emergency disallows you to keep your appointment, we ask that you
call when you can. If you do not come or call, we will automatically
bill you 25 dollars.
If three consecutive appointments are missed and you do not call,
we will offer you an alternative facility and send notice to your
doctor and insurance company that you have been discharged for
non-compliance.
